Your visit with us is coming up soon, and we want to make sure your appointment goes as smoothly as possible. You will need to register prior to being seen for your initial visit. To do so, we ask that you fill out the form below at your earliest convenience.

PRIVACY STATEMENT: Please note that no personal information from this form will be stored on our website database. All information is securely directed and saved in our clinic’s Electronic Medical Record (EMR) system where all data is encrypted. The patient information will only be used by the clinic and will not be passed on to any third party.

What should I know about my first physical therapy appointment?

  • Upon your arrival at our office, Angie will greet you at the door and welcome you to Cedar Hill Physical Therapy and Wellness Center. She will confirm your intake information and any insurance information as well. Angie will do a health screening, escort you to the exam room, and answer any questions you may have about the physical therapy treatment process. It is our intent to make it so you can focus on having the best recovery possible.
  • Paul the Physical Therapist will meet you in the exam room for your initial assessment. He will ask questions about your medical history, current pain, and/or injury during your assessment. Try to be as honest as you can; open communication helps us to understand your problem better which can lead to a faster and fuller recovery.
  • Your movement, strength, coordination, and much more will be assessed during your appointment. Our goal is to help you, so please make sure that you communicate with Paul the Physical Therapist if anything hurts or bothers you during the examination!
  • Paul the Physical Therapist will recommend and prepare a customized course of treatment to ensure the quickest recovery and the best results. Some treatments will be performed on the first day to get you started on the road to recovery. However, you will get to the full swing of your treatment plan during the next visit.
  • At Cedar Hill Physical Therapy and Wellness Clinic, we believe it is extremely important for the therapist and patient to have a collaborative relationship. Based on Paul’s assessment and your needs, you will decide together what the best course of treatment will be. We recommend that you schedule a series of visits with Paul before leaving the clinic. Please let us know if you would like text, voice, and/or email appointment reminders and the best contact for that.
  • Soon after your visit, Paul the Physical Therapist will write up a formal evaluation and plan of care to be submitted to your referring physician if there is one or any other medical professional you choose.

We offer many convenient appointment times, but prime times can be busy, so it’s best to book ahead as much as possible especially if you need a specific day and/or time.

How can I prepare for my first appointment?

  • It’s a good idea to make a list of questions that you have to discuss on your first visit.
  • List out signs and symptoms that can be of importance to the therapist. What has been hurting or bothering you? For how long?
  • Have in writing key past medical history, previous treatments, current medications, and other important information.
  • Please call our office at (336) 644-9661 to verify your insurance benefits beforehand.
  • Bring a copy of your insurance card and a photo ID with you. Likewise, you may email or text an image of your insurance card and photo ID.
  • We strive to make it as easy as possible for you to furnish us with all pertinent information prior to your first visit, so there is no need to arrive early. We understand that your time is valuable.
